Care Management Services Print E-mail
Area Agencies on Aging (AAA) often provide Care Management services, which are designed to help frail older adults who choose to remain in their homes and receive in-home care and services – thus avoiding or delaying the need for alternate placement. In-home services may include personal care, homemaking, and/or respite care. Participants must be age 60 or older.

After a preliminary qualification screening, an in-home assessment is conducted by a care manager to identify needs, existing services, and available resources. With client approval, the care manager will arrange and monitor specific services and create a plan of care. Depending on the policy of the individual AAA office, some services are offered under state and federal funding and other services by client cost-share or suggested donation.
